The NYC Department of Health and Mental Hygiene (DOHMH) is one of the world’s largest public health agencies. Its headquarters at 125 Worth Street is a historic civic building in the heart of Lower Manhattan. The agency is responsible for protecting and promoting the health of 8 million New Yorkers. This location serves as the administrative nerve center for disease surveillance, environmental health regulations, and mental health policy. Vital Records Services
The Office of Vital Records located here is the primary location for obtaining official NYC certificates. Note that orders are often processed via kiosks or online pickup windows.
