Overview
Institution-Related Eligibility District (IRED)
Specialized Medical Assistance
The Institution-Related Eligibility District (IRED) is a specialized unit within the Allegheny County Assistance Office system, located at Piatt Place in downtown Pittsburgh. Unlike improved community-based welfare offices, this department focuses specifically on determining financial eligibility for Medical Assistance (Medicaid) for individuals residing in long-term care facilities, such as nursing homes, intermediate care facilities, and rehabilitation hospitals. The staff here works closely with hospital social workers, facility administrators, and family members to navigate the complex financial requirements of long-term care coverage.
- Long-Term Care (LTC): Processing applications for coverage of nursing home care and home-and-community-based services (HCBS waivers).
- Inpatient Eligibility: Determining Medicaid eligibility for patients currently admitted to acute care hospitals who may need ongoing support.
- Spend-Down Assessments: Evaluating assets and income to determine patient liability and ”spend-down” requirements for medical coverage.
- Spousal Impoverishment: Protecting the assets of community spouses while ensuring the institutionalized spouse receives necessary care.
Service Delivery Model
The IRED office operates primarily through correspondence and coordination with medical institutions, though it maintains a public-facing office at Piatt Place. Due to the nature of the clientele (often hospitalized or in nursing care), much of the business is conducted via phone, fax, or designated representatives. However, family members acting as power of attorney often visit to submit documentation regarding assets, bank statements, and income verification. The office is part of the Pennsylvania Department of Human Services network and adheres to strict state guidelines.
