The Pennsylvania Department of Health’s Division of Vital Records operates a public branch office in downtown Pittsburgh on Seventh Avenue. This office is the primary location for residents of Western Pennsylvania to obtain certified copies of vital events. Services Offered
The Pittsburgh office provides walk-in services for the following:
- Birth Certificates: Issuance of certified copies of birth certificates for individuals born in Pennsylvania from 1906 to the present. These documents are typically processed while you wait, provided the applicant has proper identification and eligibility.
- Death Certificates: The office maintains death records for the City of Pittsburgh from 2004 to the present. For deaths occurring outside of Pittsburgh or prior to 2004, the office can accept applications to be forwarded to the central office in New Castle, PA.
Application Requirements
Applicants must adhere to strict guidelines to receive records:
- Eligibility: Only the subject of the record (if 18+), parents, legal guardians, or authorized representatives (with proof) may apply.
- Identification: A valid government-issued photo ID (driver’s license, state ID, passport) is required. If unavailable, alternative documentation is needed.
- Payment: The office accepts checks or money orders made payable to “Vital Records.” Cash and credit cards are typically not accepted at the service window.

Located at 411 Seventh Avenue, Suite 360, the office is convenient to downtown commuters. The facility operates during standard business hours, Monday through Friday. Visitors should arrive well before closing time to ensure their request can be processed. Security personnel may screen visitors upon entry to the building.
